When his Guards hesitated at the engagement of Kolin on June 18, 1757, Frederick The Great uttered his famous exhortation, "Rascals, would you live forever?" This question may be applicable today, though with a different meaning. Contemporary psychiatry is locked in a dreadful struggle attempting to balance reduced resources with growing patient needs. In the process, long cherished assumptions are being challenged and, when they cannot be shown to be of value, ruthlessly discarded.
